Coded QH-D. 0500hrs 13/07/44 accident after taking off from Tadji Airstrip PNG to carry out a night harassing raid, in co-operation with US Navy PT Boats along the Wewak coast, NG to sweep Japanese barges.  A/c had taken off with thermal overload switches for propellor in "off" position, with airscrew staying in fine pitch with RPMs building up from 3000 to 3200rpm. On return, the same switches were not checked, and a/c flew approximately for 15 minutes waiting for flare path to be switched on before making a short approach, a firing the emergency undercarriage down which failed to lock before touch down. Undercarriage collapsed and a/c bellied in. (NS = Night Search Equipment fitted, Yagi Antenna Radar).  Pratt and Whitney Twin Wasp S3C4-G #17503 and #16435. 11/02/43 Rec SHQ Nowra ex 1AD. 04/07/43 Transfer 6OTU. 11/02/44 Accident on landing at Nowra after a night torpedo exercise, a/c swung to port on flare path. Pilot tried to apply brakes, but lever on control column was jammed. Despite several failed tries, pilot decided to apply throttle instead, but that was too late and a/c swung off runway and went through a large ditch and a row of concrete pipes. Both undercarriages were snapped off. Additionally stern section torn off and port mainplane, fuselage and centre section damaged. TThrs 218.30. Operational loss on night of 14-15/11/43 when this a/c took off from Vivigani Strip loaded with a torpedo with another eleven Beauforts engaged in a combined bombing and torpedo attack in and around Rabaul Harbour. no communications were received during the operations, though army spotters on Kawa Island, did note an a/c in trouble which crashed into the sea nearby. A later investigation by patrol boats and a/c recovered a petrol tank which was identified as being from the missing a/c. Crew: Pilot: F/Sgt Jack Carroll Woodgate Serv#420730, Nav (B)  F/Sgt Stewart Ian Drakes Serv#427073, WAGs F/O Geoffrey Stewart Wells Serv#410824 and F/Sgt Byron Veitch Maclean Serv# 414053 were missing presumed killed. Write off per File#09/16/1222 Minute #4 dated 8/11/43. W/O Pratt and Whitney Twin Wasp S3C4-G #17509 and #17529. Note: RAAF was advised during January 2000 by a commercial dive operator that he had found the wreck in 15 metres of water near Kawa Island. The RAAF positively identified the a/c and removed the remains of the crew during Operation Kawa 111 (28/02/2001 to 15/03/2001 at location 08.32.97South/150.19.10 East). They were eventually interned with full military honours at Bita Paka War Cemetery, Rabaul, New Britain on 02/05/2001.

At 1330hrs(1400hrs AEST), a column of water was noticed 6 miles from convoy, accompanied by a orange flash and a dull thud somewhat resembling a depth charge explosion on bearing green 45 (starboard quarter in nautical parlance). At 1615hrs, the a/c was 45minutes overdue and the AOR No 9 Group was asked to transmit in code "What is your ETA?" this was repeated until 1956hrs by several locations (Townsville/Vivigani/Horn Island/and cairns) No answer. At 0345hrs 13/07/43 AOR No 9 Group  advised receipt that a SOUPAC PB4Y-1 aircraft ( VD-1 Photographic Squadron One, US Navy based at Carney Field, Guadalcanal) Bu No 31982 #82 "Satan's Wagon"  under command of Lt (jg) William G Corbett and crew had sighted and shot down with nose and top turret 0.50cal gunfire on a unidentified aircraft in position 06º 30 South/154º.02 East at 1308Hrs 12/07/43. This coincided with approximately where A9-225 would have been on its course. The victim a/c was seen to crash land on the sea and two white man were seen in the water. A life raft, flares and food were dropped. Official report states"A large Life raft dropped, partially inflated. Raft landed 40 feet over the men. Survivors did not see it. They were clinging to part of the wing and made no effort to reach the raft. Two life jackets tied together were then dropped near men. One man swam out and got them returning to the wing section. Another jacket was dropped with note attached "your position reported to base. Will drop rations. Raft is ahead of you. Will drop smoke lights by raft. Will leave shortly due to limited gas supply". Survivors got note. One swam to raft and pulled it to companion when men were 100 feet about apart. Life jackets were dropped with kit from other raft in plane containing rations, water, first aid kit, fishing gear, very pistol, smoke grenade. Sea Calm. PB4Y-1 departed scene after again radioing. Between 14 to 17 July 1943 searches by 100 Sqn RAAF and other Allied a/c were carried out despite the turbulent weather, but nothing was found. Ten days later it was reported that these two crew (Who had survived almost three weeks now) were twice spotted during that time. First on 1 August 1943 by another crew the raft was sighted at 06º 50 South/152º.032 East. In W/O jack Coutts words per a second 100 Sqn RAAF sighting "On 6 August 1943, however, while flying in A9-307, again in same area, and in the worst of weather, I saw the dinghy, by sheer chance through a break in the swirling clouds, at position 07° 26’ S 152° 47’ E. It had drifted some 93 nautical miles further south-west in the twenty-six days. I saw three, possibly four crew, as a canopy had been rigged at one end of the dinghy, obscuring anything beneath. Very clearly, I saw Geoff Emmett standing at the other end and frantically waving. We responded with a wing waggle and made ready with survival gear to be chucked out on the sea. Unbelievably, leaden cloud descended even lower, and the sea surface was completely veiled. We circled several times, saw nothing, and then commenced a square search until we had to reluctantly return to Gurney, after 7 hours 30 minutes in the air."  A sad end. The a/c landed 1527hrs and refuelled at Mount Gambier. The a/c took off at 1733hrs, and the a/c failed to return to East Sale. Pratt and Whitney Twin Wasp S3C4-G#18985 and #17505. 31/03/43 Rec 8 Sqn RAAF ex 1AD. 0930hrs 10/07/43 a/c took off on a bombing exercise, arriving at 0950hrs at Cordelia Rocks (21 miles NNW of Townsville and about 5 1/2 miles NE of Rattlesnake Island). The first two 250lb GP Mk4 bombs were released and functioned satisfactorily. The third 250lb GP Mk4 bomb was released at 200 feet and at 210 knots, which exploded on impact. The crew felt a jar on the a/c, followed by the WAG in turret noticing oil flowing in the vicinity of the starboard wing oil cooler. Pilot climbed to 1000 feet, jettisoned the remaining 250lb GP Mk4  bomb on safe, feathered the starboard engine and returned to the Bohle River Strip on one engine for a flaps up landing. On approach, 1030hrs, an a/c was obstructing the begining of the strip, forcing the pilot to land further down teh strip, whereupon he realised there was not enough room left, so he retracted the undercarriage and ground looped the a/c. A Seagull a/c was dispatched and the two collected. On interogation, it was advised that the a/c had overshot  its base through a navigation error and had made landfall near East Cape. The Pilot comenced flying on a north westerly direction parallel to the coast under the rainy weather, when he noticed his height was 300 feet and deminishing. The a/c struct the water, and only both the pilot and the Wireless Operator escaped the sinking a/c. After an hour in the water, natives came by canoe and collected both men, to convey them to Hioge Village where they were placed in charge of Father Jennings and Father Nichollas until they were met by an American Patrol who arranged for a collection by RAAF Sea Gull. The a/c remained with the formation up to the time of the attack, but was not seen afterwards by accompanying a/c thereafter. Post war it was reported that all of the crew were taken as prisoners after safely getting ashore and then taken to the Navy POW Camp at Rabaul. With much covering up by the Japanese into post war investigations, it wasn't until 1950 that the truth came out that F/Lt G H Vincent and W/O J P Bailey were executed on 25/11/43.  F/O C W Vincent would die in prison on 14/01/44. The bodies of the three dead crew members were exhumed in August 1948 from a mass grave Matupi, near Rabaul. They were later buried at the Rabaul (Bita Paka) War Cemetery, PNG on 18/07/1950.

The pilot made no attempt to carry out a single engine procedure. The Pilot continued to try restarting the port engine when he finally decided to force land, which resulted in choosing an unsuitable field within 7 miles of Bairnsdale VIC to crash land onto. On the third circuit the undercarriage was loaded by hand pump, and without flaps lowered, the a/c touched down 2/3s down the strip. On seeing that the a/c would overshoot the strip, the pilot selected wheels up for the ground beyond the strip seemed clear. too late for he noticed a deep bank running across the western end of the strip. Pilot applied power as the a/c floated across the creek and struck the far bank, then only to settle in the water. The pilot checked immediately  with a/c squashing on all three wheels, causing it to bounce and land on port main wheel. This in conjunction with cross wind caused a swing top port. Pilot tried to correct except the a/c struck a 3-foot mound of gravel, damaging the starboard undercarriage which then collapsed. The a/c ground looped to starboard, coming to a rest at right angles to the strip. Coded UV-U. 30/08/43 Rec DAP ex 8 Sqn RAAF. 04/12/43 operational loss, when as teh lead a/c of six Beauforts (A9-262/435/439/444/316/234)detailed to carry out a torpedo strike against enemy shipping in the St George's Channel - Blanche Bay Area off the coast of New Britain.  On approach, heavy weather was encountered and the formation was broken up, with only the leading three a/c(A9-262(Quinn)/435(Sayer)/439(Finlay) changed direction northwards past the Duke of York Islands at a very low altitude, then commenced a turn to port.  A few minutes later in the poor visibility, pilot of A9-435 lost contact with the lead a/c(A9-262). The third a/c (A9-439(Finlay) sighted an enemy vessel and attacked it at 1936hrs. A9-439's attack was successful, as described by the rear turret gunner witnessed at 1940hrs. Ten seconds later a second explosion was witnessed approximately a 100 yards further east. Both fires were still visible until A9-439 rounded Cape Gazelle. On return to base, one a/c was missing: A9-262. Crew: Pilot: Sqn Ldr Noel Thomas Quinn DFC Serv#A622, Nav(B) F/O Ross Bryan O'Loughlen Serv#400662, WAGs F/Sgt Ian Lerwick Mainland Serv#408815 and Sgt Donald McDonald Serv#401714 presumed missing.  14/12/43 Write off per File#09/16/1260 Minute #1 dated 07/12/43. W/O Pratt and Whitney Twin Wasp S3C4-G#21460 and #21453. Note: Quinn and O'Loughlen were injured and then captured and delivered to Rabaul. Quinn was at Rabaul for only 16 days after capture and was flown to Japan to a Camp located at Ofuna, outside of Yokohama. He was not on the Japanese ship Rakyo Maru which was sunk. Officially he was only stated as a POW from 31/05/45 and on surrender, he was recovered by 09/09/45 when flown from (Camp 5wB at Nagata) Japan along with many USA POWs to Manila by USAAF. He advised that F/Sgt Mainland and Sgt McDonald were killed when the a/c was shot down 04/12/43. F/O O'Loughlen survived crash and picked up by Japanese and held captive at Rabaul. Sadly, O'Loughlin died while in captivity. Post war his remains were found in a mass grave at Matupi, Rabaul which included the remains of F/Lt G H Vincent and five Americans who were also members of a stated 17 prisoners taken from the Naval Hospital on 1301/44. Stories that they were placed on a ship to Japan were a lie. O'Loughlin died while in captivity. His remains and others were laid to rest on 18 July 1950 at the Rabaul War Cemetery (Bita Paka).

Pratt and Whitney Twin Wasp S3C4-G#19150 and 21456. 29/03/43 Rec BTU (Base Torpedo Unit) SHQ Nowra. 0955hrs EST 14/04/43  Sadly, this is one of the most spectacular mid-air accidents ever to be caught on film. Three aircraft from the Base Torpedo Unit, SHQ Nowra, were participating in a formation flypast for a party of accredited war correspondents* that were on a ship in Jervis Bay, near Nowra, NSW. With a Fox Movietone camera filming the aircraft flew at low level down along the port side of the ship. Once passed the camera the aircraft on the left of the formation began to break away and turned left. As it did Beaufort A9-27, flying in the middle of the formation, began a gentle climb while Beaufort A9-268, flying to his immediate right came along side. The tip of the left wing of A9-265 struck the tail plane of A9-27, which was then torn off of the aircraft as a result. While not receiving much damage Beaufort A9-265 then commenced a left turn, which ended with the aircraft rolling on to its back and plunging nose first into the sea not far from the ship. Immediately after the crippling collision A9-27's climb steepened but it was uncontrollable and the aircraft soon lost the power to stay airborne, and it too plunged into the sea nose first not far from the place were the first aircraft fell. Lowered by usingthe hand pump, all bombs were dropped. On return to Tadji, the undercarriage was lowered by emergency firing, resulting in the locking of the port starboard undercarriage, but not the starboard undercarriage. On landing the starboard gear eventually folded and collapsed causing the a/c to swerve violently to starboard, hitting a bank and coming to rest 15 feet off the strip. The hydraulic lines and emergency lines to tail strut were found to be severed by AAA. Pratt and Whitney Twin Wasp S3C4-G#19151 and #21448.  28/03/43 Rec 32 Sqn RAAF ex 2AD.   0020hrs 27/04/43, a/c took off from Camden to carry out an anti -submarine patrol from Sydney and going via the Blue route (well off the coast) to Moreton Bay, then seaward for 30 miles then returning via the Red Route (Coastal) to Sydney, then Camden. At the conclusion of 6.5 hours a request to Fighter Command based in Sydney had confirmed a/c had passed seaward after Sydney in a North Easterly direction to gain its track along the Blue route. Having done so, it was advised that the a/c very shortly afterwards turned 90º to starboard and flew in a south easterly direction for about 30 miles, at which point the a/c disappeared from radar at 0134hrs when it was more than 100 miles out to sea. This position was recorded at 33º 43' South and 152º 32 East, as confirmed by Newcastle Sector. Pratt and Whitney Twin Wasp S3C4-G#125 and #133. 27/05/43 Rec 8 Sqn RAAF ex 1AD. 1022hrs 14/06/43 whilst on practice camera torpedo attack exercise against a naval target vessel 20 miles east of Townsville FNQ, a/c was flying in sunlight on calm water which caused the pilot to misjudge his height and bounced approximately 300 yards on the sea, before forced landed. A/c sunk and crew abandoned a/c by dinghy. TTHRs 18.05Hrs. Served in Malaya 1940-1941. Air Officer Commanding Western Area Australia 1943-1945, and from 1945 as OC No 11 Group, he was present at the Japanese surrenders in Manila and Tokyo, and on Morotai in 1945. Awarded CBE (Commander of the Order of the British Empire) Brownell died on 12 April 1974 in Perth. His autobiography, "From Khaki to Blue" was published after his passing in 1978.
